Admission Requirements

(All uploads must be in image format)

A. Old Student
- Proof of identity (example: School ID(front and back), Previous Enrolment Forms, Billing Statement, Receipts, etc)

B. New Student (Original copy must be sent to the office of the registrar through courier or hand carry)
- Birth Certificate from PSA
- Form 138 (front and back)
- Certificate of Good Moral Character
Additional Requirements for Theology Program
- Statement of Calling Essay: 300 words
- Church Recommendation
- Pastor's Recommendation
- Baptismal Certificate
- Church Membership Certification

C. Transferee
- Birth Certificate
- Honorable Dismissal
- Certificate of Grades
Additional Requirement for Theology Program
- Recommendation from previous school

D. Foreign Student
- Duly accomplished Student's Personal History Statement(PHS)
- A notarized affidavit of support or notarized notice of scholarship grant.
- Authenticated student's scholastic records by the Department of Foreign Affairs or counterpart agency from country of origin
- Student passport and Birth Certificate
- Letter of Sponsorship with contact number
