Admission Requirements
A. JUNIOR/SENIOR HIGH SCHOOL (FOR NEW STUDENTS ONLY)
Photocopy of PSA Birth Certificate (4 copies with parent/guardian’s signature)
Good Moral Certificate
Original Copy of SF9/F138/ Report Card
Original Copy of SF10/F137 (claim request letter at the Academy Registrar’s Office)
ESC Certificate (for Grade 8-12 transferees from Private-ESC institutions only)
Recent 2x2 ID Picture (white background; 4 copies)
Photocopy of vaccination card (OPTIONAL; If the student is vaccinated)
Duly signed and accomplished Student’s/Parent’s Guidelines Contract Form. (Print your Contract form on a legal (8.5x13) sized bond paper for presentation and submission to the Academy Office.)
(For GRADE 7 Applicants ONLY) APPLICATION FOR ESC (Educational Service Contracting).
Latest Income Tax Return for the previous year or Certificate of Tax Exemption or Municipal Certification of Unemployment of Parents/Legal Guardian (for incoming Grade 7 only) NOTE: ESC Application Form will be provided by the Academy Registrar.

B. OLD/RETURNING STUDENTS
Duly signed and accomplished Student’s/Parent’s Guidelines Contract Form. (Print your Contract form on a legal (8.5x13) sized bond paper for presentation and submission to the Academy Office)
C. FOR FOREIGN STUDENTS
Duly accomplished Student’s Personal History Statement(PHS)
A notarized affidavit of support or notarized notice of scholarship grant.
Authenticated student’s scholastic records by the Department of Foreign Affairs or counterpart agency from country of origin
